Meetings
- PO Monthly Calls are taking place on December 15th at 11am and December 17th at 2pm.
- Holds for spring regional meetings are on everyone's calendars, and holds for the fall 2026 virtual regional meetings will be distributed within the next week.
Reminders
- Any practice who completes their practice level and physician level learning community requirement by 1/1/2026 will earn 5 extra credit points, so encourage your practices to complete these early!
- Our initial focus group on Prediabetes resource development is taking place on 12/18/2025 from 12pm-1pm. There are still a few open slots available, and practices can sign up here. Participating meets practice level and physician level learning community requirements.
- Practices who did not attend the fall regional meeting have the opportunity to view the recording and make up some of the lost points by 12/31/2025. The link to the recording is here.
- For physician level learning community requirements, we are currently accepting feedback on two tools:
VBR Reports
- Progress reports for the PCP uACR measure, PCP year 2 A1c measure, nephrology uACR measure, and endocrinology measure are available in the Admin Portal as of 12/2/2025. Please share these with your practices who may not have access.
Please note: Some of our learning community options are updated manually (dashboard logins, participation in low carb simulation, focus groups, etc.) and are typically updated once a month. If you do not see this immediately reflected in the admin portal, this is why. We do plan to update right at the end of December as well so that we can track anyone who met the requirement before the 1/1/2026 deadline for extra credit.
Please note: MCT2D staff will be working reduced hours due to the holidays between 12/22 and 01/02. We will continue to do our best to respond in a timely manner, though response times may be longer than usual.
